About disease “Intervertebral herniation”

Herniation (Hernia)
-this is any prolapsing of some viscera from cavity, where they are normally closed, accompanying by protrusion of the cavity’s sides. Intervertebral herniation is most widespread.

This pathology (intervertebral herniation) is connected with the exchange processes’ disorder, taking place in the intervertebral disc. Liquid arrival into the area of a pulposis disc core is decreased, its amortized (elastic) function is modified and disc starts prolapsing (from here – “disc prolapse”) in direction of the least resistance from a part of surrounding anatomic formations. It is more often a back-lateral or back direction. At the same time, there is also disorder of biochemical processes inside the disc. Elasting ligaments surrounding spine, (i.e. vertebras and discs) in that place, where there is a load from pressure on them, are gradually calcified (i.e. condensed at the expense of calcium salt measures in them). In that way organism tries resist to pathology. Then as critical situation growing (aggravation of changes inside the disc and increase of load), the disc’s integrity is broken (cracks appear), the ligament, adjoining to it, is strained and a part of pulposis disc core prolapses beyond its anatomic borders.

Such prolapsing part of the disc forms herniation (hernia). So, herniation (hernia) – this is formation, prolapsing beyond its natural anatomic borders. In the given case, such formation is a part of the desolated intervertebral disc.

Intervertebral herniation (hernia) development

This disease development is caused by changes of the local metabolism in the intervertebral disc. Such changes in the disc occur due to the reflex pathologic influence from the definite internal organs. As each spinal area is functionally connected with the specific internal structures (organs, body parts…), at their pathology there is a negative impulsion, directed to the disc, and that provokes to disorder of the biochemical processes in it. At the same time, usual, and especially maximal mechanical loads on disc (at the expense of loads’ lift, sharp turns, inclines etc.) promote to herniation (hernia) formation only.

Intervertebral herniation (hernia) diagnostics

Intervertebral herniation (hernia) diagnostics is based on the medical complaints’ assessment, patient’s checkup and radial diagnostics data (rent-gene, MRT – magnetic-resonance tomography, CT – computer tomography). All kinds of the radial diagnostics don’t fully replace each other and have their advantages. From the rontgenologic point of view a condition of bone formations – vertebras – is defined better. CT, having rontgenologic base, allows more exactly defining vertebral condition in different levels. Condition of discs and of non-bone tissues are better visualized during the MRT examination, than during CT. That is why MRT is exactly such diagnostics, which is preferable at the suspicion on disc herniation (hernia). As to the cost, these examinations are distributed as follows:

Intervertebral herniation (hernia) treatment

Elimination of the suffered disc and pathologic processes, connected to it, is an aim of treatment. That is why the first tactic direction of medical prescriptions – is a liquidation of the local inflammation and of pain caused by it. With this aim many doctors prescribe nonsteroid antiphlogistic medications, for instance, diclofenac natrium or indometacin or brufen. But as practice shows, such treatment in many cases has a low effect or even appears to be quite non-effective.

There are two main directions in the treatment among patients with the intervertebral herniation (hernia). These are conservative (without operation) and operative (by means of operation). Doing operation, there is an aspiration to eliminate suffered disc and to create conditions for union of two vertebras. There are big (cavernous) and small (endoscopic etc.) operations. This way is connected with some risks. That is why its prescription should be reasonable. Statistic assessment of the after-operational consequences’ results is evidence of new herniations (hernias) formation in closely located vertebral segments, and it occurs in 40-50% of all the cases. This points out on the fact, that operation is directed on the consequences’ elimination only, but doesn’t eliminate reasons, provoking to the disc herniation (hernia) development.

Direction of the non-operational system (conservative treatment) depends on a position of doctors in respect of disease development mechanism, ways of pathology solution (recovery mechanism) and of the own preferences in a choice of methods, influencing the patient. We may distinguish three main conservative directions.

Motory system makes a patient maximally actively transform his muscular trunk and stimulate immunity. This is a sphere of very hard physical exercises. At that, there is a risk of a sharp worsening in condition, at the expense of a sudden nervous compression by a new hernia’s disc propulsion (at the pressure increase on the suffered disc). In this case there is a need in conducting of the urgent operation (in the ambulance mode). Efficiency and stability in results are different.

Manual system (manual therapy, chiropractic etc.) are directed on modification of vertebral muscles’ intensity and decrease of load on disc. They may bring facilitation, but don’t have an influence on liquidation of the suffered disc and don’t eliminate pathologic process, leading to the herniation. Risk of complication depends on the specialist’s professionalism.

Physiotherapy system presents a huge spectrum of methods (preferably passive for a patient). They contain several main directions such as: reflex (classical, traditional and modern acupuncture, massage…), field’s and oscillatory (by means of electric, magnetic, electro-magnetic fields, ultrasound, light…) and mechanical (extensions, physical culture). Efficiency depends on many factors and peculiarities.

I am sure, that valuable recovery at this disease is possible at the complex approach. The tasks of recovery are as follows:

  1. Anaesthetization (when it is needed);
  2. Antiphlogistic treatment;
  3. Renewal of the internal structures’ condition, connected with the staggered vertebral area;
  4. Renewal and correction of the immune system’s condition, the cells of which directly take part in utilization of the destroyed and mortified disc parts;
  5. Renewal of harmonic inner-system interrelations and interconnections of vertebral formations;
  6. Correction of the organism’s inner sphere (environment), having effect on the general and local metabolism, and filling it up at the deficiency.

Natural “resolution” of the staggered intervertebral disc, vertebral rapprochement and union (i.e. by forces of the organism itself) is a result of a non-operational renewal. An opportunity of such resolution is almost not reflected in researches and publications of the native specialists. This circumstance has its explanations. Nevertheless, for instance, results of the American doctors’ observations confirm organism’s orientation to such outcome:

Distance between the third and fourth vertebras (L3-L4) is considerably reduced (at the expense of the transformed disc).

Osteophytes along the edges of the vertebral bodies are the most extensively expressed in that place, where the inter-vertebral disc is almost utilized. Osteophytes present the “bone growth” along the edges of the vertebral body. At the expense of these formations there are places of the initial contact of two neighboring vertebras. As the utilization (by immune cells) and transformation of the staggered disc take place, vertebras “aspire” to unification and the immovable complex formation. This is practically not reflected on a daily, common person’s mobility. This is one of the local manifestations of the renewal at the inter-vertebral disc loss. The organism itself aspires to it; this is the only possible resolution of the Nature in this case.

The organism really aspires to help itself. That is why on radiograph you may see adaptive osteophytes’ formation (in private life – “salt measures”), creating an additional support for the load’s reduction on disc (drawing 1). These osteophytes become by the initial sites of vertebral joining after the immune system’s cells utilize the most part of the staggered disc, and the remaining part is calcified (i.e. becomes solid as a bone at the expense of calcium).
